Carrier Notes

• Agents selling in Connecticut (CT) will sell policies issued by Centrian rather than SBLI. The same writing number will be used.

Non-Resident State License Process

SBLI uses JIT contracting for all states except PA, OR, UT, IN. Please follow the below template to add your non-resident license(s) with them.

To request additional state appointments, agent should contact SBLI licensing directly VIA EMAIL as follows:

Email to: quilityagentservices@sbli.com

Subject: ‘Quility – Agent Contract Code – Agent name – Request Type’

(e.g. Quility – 12345 – Michael Smith – New Appointment)

Body: details of the request in the body of the email and attach any documents (if applicable).
